Introduction
This Privacy Policy describes how First Health Enrollment, LLC (“we,” “us,” “our”), operating golifequote.com (the “Site”), collects, uses, and shares your personal information when you visit the Site or submit information through our forms. By using the Site, you consent to the practices described in this Privacy Policy.
GoLifeQuote is a privately owned website operated by First Health Enrollment, LLC, an independent, licensed insurance agency. We are not an insurance carrier and are not affiliated with, endorsed by, or operated by any government agency or program.
Information We Collect
We collect the following categories of information:
Personal Information You Provide:
- Full name (first and last)
- Phone number
- Email address
- ZIP code
- Age range
- Coverage interest (the type of life insurance or coverage goal you select)
Information Collected Automatically:
- IP address
- Browser type and version
- Device type and operating system
- Pages visited and time spent on each page
- Referring URL and exit pages
- Cookies and tracking identifiers (see “Cookies & Tracking Technologies” below)
Consent & Tracking Data:
- TCPA consent status and consent version identifier
- Consent timestamp
- Advertising click identifiers (Google Click ID, Microsoft Click ID)
- UTM parameters (source, medium, campaign, content, term)
How We Use Your Information
We use the information we collect to:
- Connect you with a licensed insurance agent from First Health Enrollment, LLC, or one of its appointed insurance carriers, who can discuss life insurance options with you
- Process and fulfill your request for information about life insurance coverage
- Detect and prevent fraud, bot activity, and duplicate submissions
- Analyze website usage to improve our services and user experience
- Measure advertising effectiveness and attribute conversions to the correct advertising campaigns
- Comply with legal obligations, including maintaining records as required by the Telemarketing Sales Rule (TSR) and other applicable regulations
SMS / Text Message Marketing & Telephone Calls (TCPA Consent)
When you check the consent box and submit our quote form, you provide your prior express written consent under the Telephone Consumer Protection Act (TCPA) to receive marketing and customer-service text (SMS/MMS) messages from First Health Enrollment, LLC and its licensed agents at the mobile number you provide, including messages sent using an automatic telephone dialing system (autodialer) or other automated technology, even if your number is listed on a state or federal Do Not Call registry. Checking this box is not a condition of any purchase or quote, and it does not provide consent for autodialed or prerecorded/AI-voice marketing telephone calls.
Consent is not a condition of any purchase or of receiving a quote. Checking the consent box is optional — you may submit the form and request information without agreeing to receive autodialed marketing text messages. If you do not check the box, we will not record SMS marketing consent for your submission; a licensed agent may still contact you about the inquiry you submitted, including by phone and email at the number and address you provide.
Message frequency varies. Message and data rates may apply. You can opt out of text messages at any time by replying STOP to any message you receive from us; after you reply STOP you will receive a single confirmation that you have been unsubscribed, and we will send no further marketing texts to that number. Reply HELP to any message for help, or contact us at (844) 569-1969 or info@firsthealthenroll.org. Opting out of text messages will not, by itself, stop telephone calls; to stop calls, tell the agent you wish to be placed on our internal Do Not Call list or contact us using the methods in “Your Rights & Choices” below.
We retain a record of your consent (including the consent text, a timestamp, and the consent status) as described in “Consent & Tracking Data” and “Data Retention.” You may revoke your consent at any time using any reasonable method, including the STOP keyword, the methods described in “Your Rights & Choices,” or our Contact Us page.
SMS/text-message consent is never shared, sold, or rented. The mobile phone number you provide and your SMS/text-messaging opt-in and consent are used solely by First Health Enrollment, LLC and its licensed agents to contact you about your inquiry. This SMS consent information will not be shared with any third parties or affiliates for marketing or promotional purposes, and is excluded from any “sale” or “sharing” of personal information described elsewhere in this Privacy Policy, including the state-specific sections below.
Information Sharing & Disclosure
We share your personal information with the following parties for the purposes described below:
